Job objective: Provide on-site service to Cathedral residences including stabilization case management, housing advocacy, crisis intervention, information, support and security


Duties:

Maintain or monitor waitlist where appropriate
Conduct assessment, intake and screening of prospective residents in coordination with other providers
Service as a liaison with the Boston Housing Authority and provide assistance to eligible individuals or families throughout the application process. This includes: completing applications, accompanying them to interviews, and following up on the status of their applications.
Act as a liaison between clients, BHA administration and property management staff, medical personnel, legal advocacy, AAC staff and volunteers. Balance advocacy for residents and family members to ensure safe, decent and suitable housing.
When necessary provide assessment and reassessment of client’s bio-psychosocial status and needs including the following areas; health and medical (including ID and / or PCP care), financial and legal, nutritional, housing, transportation, support and mental health, substance use, and sexual health.
Develop an individualized service care plan, (up to 6 months) to reflect client needs based on assessment or reassessment.
Monitor client progress through face to face meetings and phone calls with clients.
Coordinate client’s care by working in conjunction with client’s provider’s inside and outside of AAC. Help clients obtain and maintain medical care.
Arrange and attend home visits, doctor visits and case conferences as needed. Receive and respond to calls from providers, people with HIV, their families, and partners in a timely, sensitive manner.
Maintain timely and accurate paper and computer records of all services requested and referral made. Provide a monthly report of your work.
Attend unit, department and agency and supervisory meetings as required.
