This individual’s responsibilities will be to provide case management to women and family members as designed in program CSAT grant. Duties include: Education, Vocation, Aftercare, Medical Care, Prevention, Intake/Assessment, and Economic & Housing.

| Responsibilities |

 |
Candidate must be able to establish and maintain referral relationships with service providers. In addition, this person may handle an independent caseload and assist other personnel by ensuring proper referral of clients to all appropriate services available and necessary to them. |

| Required Skills |
|
|
|
Individual will provide intake/assessment services to clients, prepare reports, treatment plans and other necessary records, maintain responsibility for the activities of one or more groups of clients, and assist other staff members in the performance of varied work duties such as referrals and aftercare planning |

Education
and
Experience |
|
| • Bachelor’s Degree with two (2) years experience
• Strong oral and written communication skills needed
• Valid driver’s license |
